On 27 October 2026, FDA published a draft guidance on electronic submission templates for IDE applications, jointly issued by CDRH and CBER. Comments are due 28 December 2026 under docket FDA-2026-D-11188. The draft describes existing eSTAR/PreSTAR resources only, is non-binding, and does not make PreSTAR mandatory for
On 27 October 2026, the U.S. Food and Drug Administration (FDA) posted a new draft guidance on CDRH New: Electronic Submission Template for Investigational Device Exemption (IDE) Applications. The subtitle identifies the intended audience as sponsors and FDA reviewers, and the document is jointly issued by the Center for Devices and Radiological Health (CDRH) and the Center for Biologics Evaluation and Research (CBER). The version month shown on the guidance page is October 2026, and the status is draft.
What the draft guidance covers
The scope is clearly defined: IDE applications submitted to CDRH or CBER that are not exempt. The document introduces the electronic submission resources that are already publicly available, together with the associated content requirements, so that sponsors know which tool to use and how to structure their materials.
In the accompanying statement, FDA positions the document as one of several steps toward fulfilling its commitment to develop biological submission templates. The purpose of a template is to serve as a guided submission-preparation tool that makes submission content more consistent and the review process more efficient.
Draft status only
Two statements at the top of the guidance page should be noted verbatim: the document is not for implementation, and it contains non-binding recommendations. This version is issued for public comment only.
According to the text, once the guidance is finalized, FDA will further address the submission standards for electronic format, the timetable for establishing those standards, and the criteria for exemptions and waivers. In other words, the version published today creates no new mandatory obligations and does not provide a final format specification that sponsors can implement immediately.
Comment period and docket number
The deadline for submitting comments is 28 December 2026. The docket number is FDA-2026-D-11188, which can be searched on regulations.gov. The address for written comments is given in the guidance: Dockets Management, Food and Drug Administration, 5630 Fishers Lane, Rm 1061, Rockville, MD 20852. All written comments must reference the docket number.
On the Federal Register side, the corresponding document number is 2026-21953, and the public inspection page indicates publication is scheduled for 28 October 2026. The officially published version will govern.
What the templates look like today
FDA's eSTAR Program page lists three separate templates: eSTAR 7.1 for non-IVD devices, eSTAR 7.1 for in vitro diagnostic devices, and the PreSTAR 3.1 template for early submissions. The third is the template that covers IDE.
PreSTAR 3.1 is described as applicable to Q-Submissions, IDEs and 513(g) information requests, with a single template used for both non-IVD and IVD products. Its stated use is limited to original IDE applications and their supplements, as well as amendments to those applications, and expressly excludes IDE reports.
The boundary between mandatory and voluntary use is also stated on the page. 510(k)s, De Novo requests and dual 510(k)/CLIA-waiver IVD submissions fall within the scope where eSTAR use is required. IDE, PMA, Q-Submission and 513(g) submissions are listed as voluntary.
Hard limits on submission channels
- For submissions through the CDRH Portal, a single attachment is limited to 1 GB and the complete eSTAR package to 4 GB; submissions exceeding these limits must be sent by mail in accordance with the portal instructions.
- For submissions through CBER's FDA Electronic Submissions Gateway (ESG), the limit is 100 GB.
- Attachment file names are limited to 124 characters; longer names are rejected directly by the template.
These figures are unchanged by the publication of the draft guidance, but they will determine how an IDE package is split and structured.
AIMEILI regulatory interpretation
The key point for industry is the distinction between what FDA is proposing and what is currently required. This draft guidance is a consultation document issued under docket FDA-2026-D-11188: it catalogues existing electronic submission resources and template content, carries the standard non-binding disclaimer, and explicitly states that it is not for implementation. No new mandatory requirement takes effect on 27 October 2026, and no final electronic format specification is available yet. FDA has indicated that the electronic format submission standards, the timetable for establishing them, and the criteria for exemptions and waivers will be addressed after the guidance is finalized.
Equally important is the scope boundary confirmed on FDA's eSTAR Program page. Mandatory eSTAR use currently covers 510(k) submissions, De Novo requests and dual 510(k)/CLIA-waiver IVD submissions. IDE, PMA, Q-Submission and 513(g) submissions remain in the voluntary category, and PreSTAR 3.1 — the template relevant to IDE — is therefore still a voluntary tool. Sponsors who prepare IDE materials in PreSTAR today are adopting the format ahead of any requirement, not discharging a compliance obligation. The 1 GB attachment / 4 GB package limits for the CDRH Portal, the 100 GB limit for CBER's ESG, and the 124-character attachment name limit are operational constraints that apply now and should be planned for regardless of the guidance's final status.
Business impact
- For device companies running U.S. clinical trials: an IDE is an unavoidable step, and the value of templating lies less in the forms themselves than in the predictability of the submission structure — the same set of questions and the same attachment locations, so reviewers can locate content and sponsors are less likely to omit items. Voluntary early adoption of PreSTAR 3.1 can reduce rework later, provided it is not presented internally as a regulatory mandate.
- For regulatory affairs teams: the 28 December 2026 comment deadline is a genuine opportunity to shape the final requirements, particularly on electronic format standards, the implementation timetable, and the criteria for exemptions and waivers. Companies with high IDE submission volumes or complex IVD trial programmes should consider filing comments under docket FDA-2026-D-11188.
- For distributors and CROs: no immediate change to current IDE filing practice is required. Contracted submission support should nevertheless account for the portal and gateway size limits and file-naming constraints, since these determine how a package is assembled and delivered.
- Monitoring actions: track the Federal Register publication of document 2026-21953 (scheduled for 28 October 2026), the finalization of the guidance, and any subsequent change to the mandatory/voluntary classification of PreSTAR for IDE submissions. Any future shift of IDE into the mandatory eSTAR scope would be a material planning event for sponsors with pending or upcoming IDE applications.
